Medical

  • PPO National Health Care plan
  • No referrals needed
  • No Primary Care Physician required
  • Discount vision plan included
  • Monthly premiums for individuals covered in full by Centers
  • Prescription drug plan included

Dental

  • Covers 100% of in-network preventative care
  • Covers 50% of orthodontics with a $1,000 lifetime maximum
  • Yearly benefit maximum is $1,500 per insured
  • Monthly premiums for individuals covered in full by Centers

    Vision

    • Optional vision coverage
    • $20 co-pay for annual vision exam
    • Allowance for contact lenses or glasses every 12 months
    • Average of 15% off regular price of laser vision correction

    Flexible Spending Account

    Employees can set aside pre-tax money for medical and dependent care expenses up to a maximum of $5,000/year for each plan.

    Disability Benefits

    • Short-Term Disability
      • Pays 60% of weekly earnings to a maximum of $1000/week.
      • Benefit period of up to11 weeks.
      • 14-day waiting period.
    • Long-Term Disability
      • Pays 60% of basic earnings to a maximum of $6000/month.
      • Benefits payable to age 65.

    401(K) Plan

    • Eligible employees may contribute after (1) one month of employment.
    • Employees can contribute up to 100% of salary to a maximum of $16,500/year.
    • CENTERS contributes 8% on an annual basis regardless of employee contributions.

    Commuter Benefits

    Options at applicable sites for employees to pay for transit fare with pre-tax income.

    Paid Leave

    • Vacation
      • Rates are accrued based on title and tenure.
    • Sick
      • 40 hours are accrued per year.
    • Holidays
      • 8 observed holidays (see site director for exact dates).
      • 4 floating holidays.
    • Maternity/Paternity Leave
      • 2 weeks paid leave for the care of a newborn or newly adopted child.
